Your sisters children are her responsibilty. She chose to have them, not you. Dont let anyone bully you into something you cannot handle. Even if you didnt have anxiety this is not your problem.

And you do not have to make excuses either or tell them about your problem. Just say no.

You need to get the panic attacks dealt with. It's hindering your life. You don't have to help your sister, but if you'd like to then you need to start getting help for your panic attacks. It's okay that they happen, because they are something that you can't control. Have you ever thought of seeking help from your family, your mom may seem unsympathetic, but she may see it differently then you think. Ultimately, whether you need to help your sister or not, you should get yourself help..because you simply cannot keep living with panic attacks...it's going to hurt you.

You should seek medical help for your panic attacks. A lot of people suffer from anxiety and panic attacks so you are not alone.
Maybe you should tell your mother, you might be surprised and find her supportive.
Your number one responsibility is to yourself hence you need to find ways of dealing with the panic attacks before they take control of you.

there are alot of people out there that have panic attacks. please don't feel ashamed or alone. you can talk to your doctor about this. he may recommend therapy and/or meds to help you. put yourself first and try to get help for it. panic attacks can reach high levels and you shouldn't worry about anyones needs right now except yours. your sis chose to have children , she needs to step up and take the responsibility for it. after you talk to your doc, then explain the situation to your mom. it wouldn't be such a good thing for you to have an attack driving, for your safety as well as the kids. goodluck
